New clients and returning clients who need to update their contact information, please complete the following form prior to your appointment to assist with the efficiency of your appointment.

Please bring the following items and information to your appointment:

RECORDS:
Any records containing medical information and treatments NOT provided by Oceana Veterinary Clinic or White Lake Animal Clinic.

MEDICATIONS:
List of medications and supplements your pet is currently taking, including heartworm prevention and flea/tick prevention.

FECAL SAMPLE:
Please collect and bring a small "tootsie roll" size of poop.  Fecal samples should be collected within 24 hours of your scheduled appointment.  We recommend intestinal parasite testing at least once annually to help protect your pet and your household.
